Programs and Services
Our staff works closely with you to review your concerns, answer questions, provide referrals and to support you through a specific life event or issues such as:
- A health crisis, temporary home care, meals delivered
- Loss of a loved one
- Applications for affordable senior housing or benefits
- A fire, a flood, eviction
- Loss of income due to an accident
- Assistance preparing advanced directives
- Energy Assistance- utility payments
Financial Assistance is available to eligible people for Rent, Prescriptions, Eyeglasses, Hearing Aids and other services to improve quality of life, promote health and help people remain connected and safe in their community. An appointment for a review of concerns and eligibility for benefits, grants or other services, can be scheduled in the office or in your home if your are unable to travel outside your home.
In some cases, a referral to specialized care is all that is needed. In many cases however, a more comprehensive plan is developed and we provide guidance, coordinate with a network of agencies and professionals to create a safety net and solutions.
When a life change makes decision making difficult, help is available to support you as you review options and make considered choices for you and your family.
Short term personal counseling offers a chance to express difficult emotions and learn new coping strategies for dealing with change, anxiety, health, loss or social and family issues.
Our community outreach program includes home visits and conversation groups where older adults share their experiences with peers with guided support. Current groups meet at senior housing sites throughout Stamford. Through this program we often identify people who need additional home care, or financial aid, and prompt referrals and support is provided.
Additionally, our outreach coordinator makes visits to health care facilities, hospitals and clients homes during times of need or to meet with people who are unable to get out to meet in a group setting.
